Privacy policy
How the practice collects, uses, stores and shares your personal and health information under the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
This policy applies to East Hills Medical Centre Pty Ltd, trading as East Hills Medical Centre, at Shop 1, 17 Maclaurin Avenue, East Hills NSW 2213. A printable version is available as a PDF and at reception.
What we collect
Your name, date of birth, address, contact details, Medicare and healthcare identifiers, concession and DVA details, private health fund details, emergency contact and next of kin, and your health information: history, consultation notes, results, referrals, medications, allergies and immunisations. Cultural background may be collected to support appropriate care.
Why we collect it
To provide and coordinate your care, to communicate with other treating providers, to claim Medicare, DVA, WorkCover and other benefits, to send reminders and recalls, to meet legal obligations, for accreditation and quality improvement, and for the training of medical students and registrars under supervision.
How we collect it
Directly from you in person, by phone, on forms and through online booking; from other providers such as hospitals, specialists, pathology and imaging services, and My Health Record; and from Medicare and DVA where authorised.
Who we share it with
Other health providers involved in your care, with your consent or as reasonably expected; pathology and imaging services; Medicare, DVA and insurers for billing; My Health Record where you have not opted out; the practice's software and secure messaging providers under confidentiality agreements; and where required by law, such as mandatory reporting or a court order. We do not sell or disclose information for marketing.
Storage and security
Records are held in a secure electronic practice system with access controls, audit trails and backups. All staff sign confidentiality agreements that continue after employment ends. Paper records are held in locked storage. The website contact form is for general enquiries only and should not be used for health information.
Access and correction
You may request access to your record or ask for a correction by writing to the practice manager. Requests are actioned within 30 days. A fee may apply for copies or transfer of records ($60 for a full transfer).
Website
The website does not use advertising or tracking cookies. Server logs record IP address, pages visited and browser type for security. Online booking is provided by HotDoc and HealthEngine under their own privacy policies.
Complaints
Contact the practice manager at info@easthillsmedical.com.au or (02) 9773 3676. If unresolved, you may complain to the Office of the Australian Information Commissioner (oaic.gov.au, 1300 363 992) or the NSW Health Care Complaints Commission (1800 043 159).
